The need for such a service is especially compelling because dermatology is already recognized within the university hospital’s academic and clinical structure since the establishment of the Department of Dermatovenereology in 2007 G.C by Dr. kassahun Bilcha . These developments show that dermatologic care has already taken root institutionally; however, the absence of a dedicated dermatopathology service leaves an important diagnostic gap. (3,4)     

At present, there is no dermatopathologist and no specialized dermatopathology section at the hospital. This limitation has serious implications. Many skin diseases resemble one another clinically, yet differ significantly at the microscopic level. Inflammatory dermatoses, autoimmune blistering disorders, pigmentary lesions, infectious conditions, and skin cancers often require histopathologic confirmation for accurate diagnosis. Without a dedicated dermatopathology service, clinicians may be forced to rely more heavily on clinical judgment alone, empirical treatment, or delayed consultation, all of which can prolong uncertainty and affect patient outcomes. Currently, graduates from the University of Gondar (UoG) in both Dermatology and Pathology departments face a significant disadvantage due to a lack of specialized dermatopathology exposure. While these clinicians are highly skilled in their primary domains, their training in the nuanced interpretation of skin biopsies remains underdeveloped.

From a systems perspective, the creation of a dermatopathology service would enhance institutional capacity. It would support better collaboration between dermatologists and pathologists, improve specimen processing standards, and promote more specialized reporting practices. Over time, this referral center can create a setup not only for dermatologic treatment but also for advanced dermatologic diagnosis and training. In a country where specialist services remain limited in many areas, this would be a meaningful contribution to more equitable and higher-quality healthcare delivery.

The establishment of such a service does not necessarily require immediate full-scale expansion. A phased approach could begin with creating a specialized dermatopathology section within existing pathology services, strengthening biopsy handling protocols, developing regular clinicopathologic conferences, and pursuing collaboration or mentorship with external specialists while local expertise is built. Ultimately, the long-term goal should be the development or recruitment of a trained dermatopathologist who can anchor the service sustainably.

In conclusion, establishing a dermatopathology service at University of Gondar Comprehensive Specialized Hospital would have far-reaching implications. It would improve diagnostic precision, strengthen patient care, support teaching and research, and elevate the hospital’s role as a leading academic and referral institution in Ethiopia. Given the university hospital’s existing dermatology service, its educational mission, and its strategic importance in the region, the creation of a dedicated dermatopathology section is not merely desirable; it is a necessary advancement in specialized healthcare delivery.
